Does The Doctor Need To Prepare A Written Statement For The POA To Be Effective?
In Ohio we didn’t need one. But my LO was in Earl stages and she signed off on both health and financial
You will need to rely on people from your own state to get a good answer. The ADRC (Aging & Disability Resource Center) at your county seat can help you. Their services are free and they have lots of resources. In WI we don’t need a Dr letter for the POA - Financial. WI doesn’t have a POA - Health care. Ours is a Living Will and we need 2 Doctors to sign that the person is incompetent.
I didn't need a Dr to certify, in Florida. The sooner you do a POA, MEDICAL POA, LIVING WILL, while the person clearly can state their wishes, the better. Find a competent attorney.
Mine was written in SD with our wills I never had to have a Dr say he had ALZ, best to check
When I got the medical power of attorney in Kentucky, Bill was still (sort of) able to consent. Because it was during Covid, a paralegal, and another law clerk came to the house with the paperwork.
