Yesterday, I also told you that there's also the FAST Scale (Reisberg Functional Assessment Staging Test).
I explained that I wanted one that incorporated the "Instrumental Activities of Daily Living" and also the "Activities of Daily Living" -- because care providers will ask about those things -- but didn't find one anywhere online, so finally, I just made my own spreadsheet. Here's a copy:
