These are some examples that I was given when talking to a hospice company: incontinent, can't form a sentence anymore, can't dress, weight loss, and trouble swallowing. Everyone is different though. Maybe you can get an assessment, and also look into palliative care which I plan to set up for my dad as a "pre-hospice." My best to you.
